Activity Release of Liability, Waiver of Claims,
Express Assumption of Risk & Indemnity Agreement
1. This slave is freely and voluntarily choosing to view and/or participate in activities that it knows are adult-oriented and sexually explicit. it is aware that such activities involve acts of Domination, submission, bondage, discipline, sadism, masochism, and other explicit and extreme sexual fetishes including, but not limited to, spanking, paddling, whipping, piercing, knife or edge or blood play, suspension, fisting and other oral, vaginal and anal penetration. it understands that these activities involve certain risks and dangers, including the risk of injury. it is aware that simple things such as use of hormone treatments, blood pressure medication, iron deficiencies and similar illness or medications can cause a much higher risk of leaving marks and that no impact play of any kind is completely risk free from leaving marks and sometimes bruises may arise days later when they were not present previously. it voluntarily accepts full responsibility for and agrees to assume all risks involved.

2. This slave is aware that the use of BDSM related premises, facilities and equipment may be dangerous and hazardous and it hereby assumes all risk of the use of such premises, facilities and equipment. it agrees to personally inspect all the premises, facilities and equipment and make its own independent judgment as to the use thereof. it agrees not to rely upon the representations of anyone else in making such judgments.

3. As consideration for it being permitted to participate with Mistress, this slave agrees to release hold harmless and indemnify Her, any premises owners and other participants and their employees, agents, volunteers, officers, directors, contractors, members and subsidiaries from any and all claims, including claims based upon negligence, arising out of its participation. This agreement is also binding upon its family members, heirs, and executors.
4. In the event of any litigation, dispute, or arbitration arising out of this agreement or its participation with Mistress, should any portion of this agreement be invalidated all remaining portions of this agreement shall remain in full force and effect.
